Pericardiocentesis / Pericardial Window
SOC → SCF-DBI Logic Translation
Purpose
Pericardiocentesis and Pericardial Window are procedures performed to diagnose and/or relieve pericardial fluid accumulation that is impairing cardiac filling and causing hemodynamic instability.
In trauma settings, these procedures are primarily used to identify and treat hemopericardium and cardiac tamponade.
SOC Definition
Pericardiocentesis
Clinical Objective
Remove pericardial fluid using needle and catheter access to:
- Relieve cardiac tamponade
- Improve ventricular filling
- Improve cardiac output
- Stabilize the patient before definitive intervention
Pericardial Window
Clinical Objective
Create a surgical opening into the pericardium to:
- Confirm hemopericardium
- Drain blood or fluid
- Assess for cardiac injury
- Guide need for sternotomy or thoracotomy
SCF-DBI Translation
Core Concept
SOC views these procedures as:
Pericardial decompression procedures.
SCF-DBI views them as:
Neurocardiac Perfusion Restoration Procedures.
The primary pathology is not simply fluid accumulation.
The pathology is progressive interruption of:
- Cardiac filling
- Cardiac output
- Coronary perfusion
- Cerebral perfusion
- Neurocardiac synchronization

Domain 2
Neurocardiac Perfusion Recovery Index
Major SCF-DBI Enhancement
Selected Enhancement:
Add Neurocardiac Perfusion Recovery Index
This becomes the primary SCF enhancement beyond traditional tamponade relief.
Rationale
SOC success is often defined by:
- Blood pressure improvement
- Tamponade relief
- Fluid drainage
SCF-DBI evaluates:
Recovery of the Neurocardiac Perfusion Axis
because restoration of circulation does not guarantee restoration of cerebral perfusion.
