Surgical Airway
SOC → SCF-DBI Logic Translation
Purpose
A Surgical Airway is an emergency airway access procedure performed when conventional airway management has failed or is impossible.
The procedure establishes a direct airway through the anterior neck to restore oxygen delivery and prevent hypoxic death.
SOC Definition
Clinical Objective
Create immediate airway access when:
- Endotracheal intubation fails
- Oxygenation cannot be maintained
- Upper airway obstruction prevents ventilation
Common Indications
Cannot Intubate – Cannot Oxygenate (CICO)
Complete airway failure despite:
- Bag-mask ventilation
- Supraglottic airway attempts
- Endotracheal intubation attempts
Upper Airway Obstruction
Examples:
- Facial trauma
- Laryngeal trauma
- Massive neck swelling
- Thermal injury
- Anaphylaxis
- Foreign body obstruction
Airway Destruction
Examples:
- Penetrating neck trauma
- Maxillofacial disruption
- Laryngotracheal injury

Core Concept
SOC views Surgical Airway as:
Emergency oxygenation access.
SCF-DBI views Surgical Airway as:
Neurovascular Oxygen Delivery Network Rescue.
The primary threat is not merely airway obstruction.
The true threat is progressive collapse of the:
- Oxygen Delivery Network
- Cerebral Oxygenation Network
- Neurovascular Integrity Network
- Cellular Energy Network
